Digitalisation for hospitals

How can a district hospital in Moldova move to the electronic patient record?

By starting from the forms doctors already fill in, without changing them. The record looks as it does on paper, but it is completed from templates and by dictation, receives laboratory results and composes the discharge summary itself, which the doctor checks and signs.

In short

The direct answer

In many district hospitals doctors still write the record by hand, and the existing information system only keeps track of admissions and the pharmacy. The discharge summary is rewritten from the record, test results arrive on paper, and a new form costs money from the supplier.

We built the electronic record on the real forms of a district hospital in Moldova: the first page of the inpatient record field by field, the initial examination with checkboxes, the annexes and the patient consent forms. The demonstration runs today, with fictitious data, on six roles: registry, doctor, nurse, head of department, laboratory and director.

Going live does not require new code for each hospital. The hospital provides the staff list, the departments, the laboratory computers and the server. Accounts are activated by invitation, and a readiness screen shows what is still missing before the first working day.

What it does

From admission to discharge, in the same record.

The approved forms, unchanged

The first page of the record stays exactly the approved form. The initial examination, daily progress notes and epicrisis are completed from department templates.

Dictation in Romanian

The doctor dictates, the text appears in the record and is corrected before saving. Dictation starts only if the hospital enables it and chooses where the voice is processed.

Laboratory

Referrals reach the laboratory with the record number. Analysers that allow a connection send results straight into the record, with reference ranges.

Discharge summary

The discharge summary and the epicrisis are composed from what has already been written in the record. The doctor reads, corrects and signs them.

Step by step

A realistic order for a hospital.

01

A demonstration on your roles

Doctors, nurses and the laboratory try the record on fictitious data, on the hospital's forms.

02

Forms and departments

We adapt the templates to the hospital's departments. The first page of the record does not change.

03

Staff, laboratory, server

Staff import, activation invitations, enrolment of the laboratory computers and analysers.

04

A pilot in one department

One department works in the electronic record, then the rest, once the team says it is ready.

Rules

What we never do in a hospital.

  • The AI makes no diagnoses and signs nothing. Whatever it proposes, the doctor confirms or deletes.
  • Production starts empty: no invented patient, account or analyser. Demonstration data never reaches the hospital's server.
  • Every opening of a record stays in the log: who, when and in which role.
  • Administrators and management sign in only with two-step authentication.

Plainly put

Where others are a better choice.

  • For republican hospitals already working in a complete hospital information system, extending that system may be the simpler path.
  • For national projects, with multi-year public procurement and integration with every state system, large integrators have teams and references to match.
  • If the hospital already has an electronic record that doctors use, it is worth completing, not replacing.

Questions

Frequently asked questions

Do we have to give up the information system we have?

Not necessarily. The record can work alongside the existing system or replace it in stages. The hospital decides, after the demonstration.

Where is patient data stored?

On the hospital's server. Production is installed separately from the demonstration and starts without data.

Who can see a record?

Only the roles entitled to, for example the attending doctor and the department. Every opening is recorded in the log.

Do doctors have to learn a new program?

The record follows the forms they already fill in. The application has a tour for each role, explanations on fields and printable guides.

How much does it cost?

It depends on the number of departments, the laboratory analysers and what already exists. We set the price in a staged, fixed-scope offer, with no fee for each new form.

Can we see the record before any decision?

Yes. We run a demonstration on the hospital's roles, with fictitious data, on site or online.
